The Wallace Health Centre will be OPEN

HOWEVER Burney Street surgery will be CLOSED

Both sites are CLOSED for Bank Holidays 

Tuesday 25th Christmas Day, Wednesday 26th Boxing Day & Tuesday 1st New Years Day

Prescription requests received after 4pm on Friday 21st will NOT be ready until after 4pm on Friday 28th December

Improving Access 

As from the 1st April 2018 the Burney Street catchment area has reduced so that we may take care of the local residents living in and around the Surgery. 

Existing registered patients will not be removed unless in the future they move home however they will be able to remain at Burney Street if they move into the new catchment area.

A list of the accepted streets can be found under the New Patients section.

Health Authority GMS1 New Patient Registration Form (PDF file)

Information for visitors from abroad

Once completed you will need to bring them with you to the surgery along with the documents below (these MUST be dated within the last 3 months). Inability to provide any of the necessary documentation please speak to Reception about your Registration.

  • One form of Photographic ID (e.g. Passport; UK Drivers Licence; Student Card)
  • One proof of address (e.g. Gas, Water, Electricity, Phone or Council Tax Bill; Bank Statement; or a letter from your landlord or housing department to confirm your residency.  This must be dated in the last 3 months)

After submitting all of the above documentation to either of our Surgeries, please allow 48 hours before making an appointment to allow for the completion of the registration process.
